Three Steps to Submit by Mail

  Create a DTIS Account

  Start by creating your secure DTIS account. You’ll use this account to complete registration, submit payment, and access your results. Create your account.

  Complete and Mail Your FD-1164 Fingerprint Cards

  Visit your local law enforcement agency (police or sheriff’s office) to have your fingerprints taken on an FD-1164 fingerprint card.

  • You’ll need two completed FD-1164 cards. Request two sets at your appointment
  • Self-inked fingerprints are not accepted by the FBI. Prints must be taken by law enforcement
  • Fill out all fields completely (name, date of birth, Social Security Number, etc.)
  • Under “Reason Fingerprinted,” write: FBI Departmental Order
  • Both you and the official taking your prints must sign the card
  • Mail one completed card along with a copy of your online registration confirmation to:

  Keep your second card on file in case the FBI rejects the first submission.

  Access Your FBI Results

  Once your results are ready, you’ll receive an email notification with a link to log in to your DTIS account.

  • You have 30 days to access your results once posted. After 30 days, they are permanently removed
  • Only access your results when you are ready to print or save them immediately
  • Prefer to receive results by mail? Select the “I do not have a phone available” option at registration
