How to Submit Your FDLE Fingerprints by Mail

If you’re unable to visit a DTIS partner location, you can still complete your Florida background check by mail. This option is available for select non-photo enabled ORI numbers only. Contact our team before getting started to confirm your ORI number qualifies.

Four Steps to Submit by Mail

  • 1

    Confirm Your Eligibility

Florida mail-in fingerprinting is available for select FDLE ORI numbers only. Before proceeding, contact DTIS Customer Service to confirm your ORI number qualifies for hard card processing.

  • 2

    Create Your DTIS Account

Start by creating your secure DTIS account. You’ll use this account to complete registration, submit payment, and access your results once they’re ready. Create your account.

  • 3

    Complete Your FD-1164 Fingerprint Card

Visit your local law enforcement agency (police or sheriff’s office) to have your fingerprints taken on an FD-1164 fingerprint card. Call ahead to confirm they offer the service and have FD-1164 cards available. Some agencies may charge a fee.

    • You’ll need two completed FD-1164 cards. Request two sets at your appointment
    • Self-inked fingerprints are not accepted. Prints must be taken by law enforcement
    • Fill out all fields completely (name, date of birth, Social Security Number, etc.). Enter “N/A” for any field that does not apply
    • Under “Reason Fingerprinted,” write your ORI number as provided by your employer or requesting agency
    • Both you and the official taking your prints must sign the card
  • 4

    Mail Your Fingerprint Card

Mail one completed FD-1164 card along with a copy of your online registration confirmation to:

    DTIS
    10201 Fairfax Blvd, Suite 470
    Fairfax, VA 22030

Keep your second card on file in case your fingerprints are rejected. Once DTIS receives your card, standard processing takes approximately five business days.
