I am asking Digital Trusted Identity Services (and its agents), to take and send my fingerprints to applicable government agencies ("Agency") for the purpose of checking my criminal history records information ("CHRI"). I authorize the release of my CHRI to the Agency. I understand my fingerprints may be forever and checked periodically with the Agency receiving CHRI updates.
In my photo was taken, my photo may be shared only for employment or licensing purposes and may be retained by the Agency. I have the right to challenge any CHRI that may be inaccurate.
I have read and agree with the Biometric Retention and Destruction Policy ("Policy") document. I understand Digital Trusted Idenity Services will retain and destroy my fingerprints in accordance with the Policy and will keep the information in this fingerprint application for up to 5 years. I understand that unless obligated by contract or applicable law, my fingerprints will be retained by Digital Trusted Identity Services for 90 days as set forth in the Policy.

If you believe that a breach of your Personally Identifiable Information (PII) has occurred as a result of our services, you have the right to report the incident to the FBI.
The FBI defines PII as:
Personally Identifiable Information (PII) means information which can be used to distinguish or trace an individual's identity, such as a name, social security number, or biometric records, along or when combined with other personal or identifying information which is linked or linkable to a specific individual, such as date and place of birth, or mother's maiden name.
The FBI defines PII breach as:
PII Breach means the loss of control, compromise, unauthorized disclosure, unauthorized acquisition, unauthorized access or any similar term referring to situations where persons other than the authorized users, and for other than authorized purposes, have access or potential access to PII, whether physical or electronic.
